The following document provides details of QLogic's PCI Fibre Channel StorMiniport driver 
changes between version 9.1.0.15 and 9.1.0.16.

9.1.0.16 QL 3.03.16 4.00.12 - 08/29/05
    *   Embed 24XX FW in the driver

III. Bug Fixes

9.1.0.16 QL 3.03.16 4.00.12 - 08/29/05
    *   Close ER43175: New driver could not detect any device to install as BOOT from SAN on 
        one specific setup.  Investigation showed that driver detects the target device successfully but 
        failed the logins because the target had previously been logged in through by the HBA firmware.  
        Driver is changed to handle this particular case correctly.
